Program Enrollment Information

  • If you have outstanding prerequisites or have not completed your degree, please send a copy of your college transcripts after completion to: ºÚÁÏרÇø of Health Professions, Attn: PA Department, 1800 South Novell Place, Provo, UT 84606. If your college allows electronic submission of transcripts, please have them sent to pa-forms@rm.edu.
  • You will be receiving an email from Certiphi to complete your background check. Please complete your background check as soon as possible. The background check cost will be paid by you.
  • After you receive your student email address from IT, please make sure that you check this email frequently. This is how we will communicate with you going forward.
  • Your second deposit of $500 will be due on Mar. 1, 2026.
  • There will be a required Canvas Course (ºÚÁÏרÇø 101) to complete before classes begin. Details will be sent in Mar. 2026.
  • The first day of classes will be May 4, 2026.
  • There is a required University Orientation on May 1, 2026. Information to follow.
  • You will receive a scrub voucher to Total Scrubs in Provo at University Orientation. This will allow you to purchase University scrubs. Only University scrubs are allowed to be worn to campus. However, you can wear any navy color scrub bottoms.
  • You are required to purchase a stethoscope for the program. The following are stethoscopes that meet our minimum requirements:

Post Acceptance Forms

By now you should have received an email from Enrollment Management outlining the steps that you need to take in order to be enrolled in this program.  We require that you also complete the forms listed below.*

Specific Forms

You are required to complete an Immunization Form and a Technical Standards Form that is to be signed by a MD, DO, NP, or PA. Please print off the Immunization Form and Technical Standards Form below. After they are completed, you will submit them here by scanning in the documents to your computer and uploading. There is a link to a PDF letter about the TB testing that is required prior to matriculation. You are also required to upload your TB test(s).

  1. You must have a Hep B titer. This will confirm immunity. If the immunity is negative, you will be required to complete a new Hep B series and another titer. Please use if you need a new Hep B series with titer.

  2. Our immunization form says “1 to 2-dose vaccine series depending on indication” for MMR immunization. Please note that we strongly advise that you have 2 MMR doses on your form. It is highly unlikely that the situation applies to you in which a 1-dose series may be in line with the CDC recommendations. To see when a 1-dose series may apply, please see the MMR row on the table on this CDC page: . If that does not apply, which should be most or all of you, please make sure you have the 2-dose series.

  3. For Varicella, you can either get the titer OR provide the dates of vaccinations.

  4. All others require dates of vaccinations.

  5. If you naturally contracted measles, mumps, and/or rubella in the past, you can complete that will be needed to document your immune status.


You are required to have this form completed by a provider.


You are required to complete either a two-step series of TB tests or a blood test. Please read the instructions carefully.

Program Accreditation

The Accreditation Review Commission on Education for the Physician Assistant, Inc. (ARC-PA) has granted Accreditation-Continued status to the ºÚÁÏרÇø of Health Professions Physician Assistant/Associate Program sponsored by ºÚÁÏרÇø of Health Professions. Accreditation-Continued is an accreditation status granted when a currently accredited program is in compliance with the ARC-PA Standards.

Accreditation remains in effect until the program closes or withdraws from the accreditation process or until accreditation is withdrawn for failure to comply with the Standards. The approximate date for the next validation review of the program by the ARC-PA will be March 2029. The review date is contingent upon continued compliance with the Accreditation Standards and ARC-PA policy.

Graduates of programs accredited by the ARC-PA or its predecessors are eligible to take the Physician Assistant National Certifying Examination (PANCE). Certification is required to be eligible for licensure to practice as a Physician Assistant/Associate. PA program graduates are deemed as having graduated from an accredited program if their PA educational program was accredited at the time of their matriculation.
