Degree Completion Extension Request
The Degree Completion Extension Request Form must be completed when a student is approaching their maximum timeframe for degree completion. Failure to聽submit聽this request prior to your deadline will result in dismissal from the university. Submitted requests are reviewed by the Program Director and Dean for approval.
