Re-entry Form

Submission of this form notifies the University of your intention to re-enter your program. The Re-entry Request Form must be submitted 1-3 months (but no later than one month) before the requested re-entry semester start date as published on the Academic Calendar. The Registrar’s Office will contact you with a proposed degree plan once Finance, Financial Aid, and your Program Director have signed off. If at any time you need to cancel this request, please contact the Registrar’s Office. (registrar@rm.edu or 801.375.5125 option 3).

Students in University Withdrawal (UW) status may request re-entry. However, readmittance from UW is not guaranteed and is subject to review and approval from the PD and Dean. The student may be required to re-apply to the program and/or submit additional documentation for the request to be considered, including a personal statement to address their desire to continue the program and to indicate what has changed to permit successful completion. New references must be submitted if the references originally submitted are more than three years old or no longer on file. The student must meet all admissions and programmatic requirements in effect at the time of re-application. The student will be held to the curriculum requirements and tuition amounts of the current program, not their original program. Lastly, the student will be required to submit a new Enrollment Agreement, and to review/abide by the current version of the .
